The Altar is irrelevant, in that you only need A sac engine. It doesn't matter which one.

With the Parallel Lives out, when the Reef Worm dies, you get two fish. Those two fish in turn net you a total of four whales. Those four whales in turn net you a total of eight 9/9 krakens.

Then for giggles, cast Whelming Wave to bounce everything but your kraken tokens. Not part of the combo, but amusing anyway.

Now that I look at it, Altar of Dementia would be a terror with that worm.

Get nothing from the worm, mill 3 from the fish, 6 from the whale, and another 9 from the kraken. Add in the parallel lives, and that will jump from milling 18 to 102. 3-card combo to mill an entire commander deck in moments.
