Get all but the narcomoeba out. Once you either mill it off your library or cast and sacrifice it, the fun starts:

-Sacrifice Narcomoeba to the Altar of Dementia, targeting yourself (this will result in milling a single card)
-Because a creature hit your graveyard, Mortuary throws it back to the top of your deck.
-The Altar's triggered ability resolves, putting the top card of your library, the Narcomoeba, into your graveyard.
-Narcomoeba sees its in the grave from your library and reanimates itself.
Run this through the requisite number of times, and cast Bitter Ordeal, exiling your opponent's whole deck.
